Attorney, David Stoll, was present to monitor. Ms. Donna Ferrante made a statement
on behalf of Ms. Cynthia Ferrante. After discussion and consideration, a motion was
made by Dr. Christensen and seconded by Dr. Bertch-Mumaw, to accept the
investigative Committee's recommendation and dismiss this issue with no violation.

Attorney, David Stoll, was present to monitor. Ms. Donna Ferrante made a statement

Further discussion took place and the Board directed staff to contact, consulting
veterinarian, Dr. Petcu, to discuss his submission fo the Board in response to the case.

record at a minimum of every 15 minutes while anesthesia is being administered; and

*Continuing Gentamycin to a dog with known kidney issues resulting in renal failure and
concurrent death;
*Inadequate fluid administration to a dog with a serious condition;
e Lack of adequate pain management resulting in the animal suffering;
«Not performing a urinalysis; and
*Sending the dog home when in renal failure.

. ADMINISTRATIVE REVIEW OF PREVIOUS ACTIONS
1.11-99: In Re: Audie Wood, D.V.M. Board appearance in compliance with Board Order.
Dr. Wood was present in compliance with her Board Order, made statements and
answered questions. It was brought to the Board's attention that Dr. Wood's random urine
screen showed a dilute creatinine level and specific gravity that was in the acceptable
range by .0004; out of nineteen screens she has completed, five have indicated abnormal
levels for these two indicators. Dr. Wood indicated she drinks a lot of water, which may
impact these two indicators.
2.13-33: In Re: Greg Heltman, D.V.M. Board may review, discuss and take action on
Practice Monitor Reports.

employment offer with details and has relayed her current status to the potential

Ms. Whitmore updated the Board of the timeline of events associated with the premise
application for Catalina Animal Hospital, the subsequent premise inspection and the failure
to submit corrections of the violations found at the premise inspection. Ms. Whiimore
narrated a picture presentation of the premise inspection that had occurred. After
discussion and consideration, a motion was made by Dr. Serbin and seconded by Dr.
Berfch-Mumaw to deny Catalina Animal Hospital's premise application pursuant to:
« ARS. § 32-2274 (A) (2), (3) as outlined in the Board staff memo, (4), (5) and (6); and
e =A.R.S. § 32-2232 (13) fraud or misrepresentation in procuring a license.

communities for veterinary students participating in the Western Interstate Commission for
Higher Education (WICHE) Professional Student Exchange Program.
After discussion and consideration, a motion was made by Dr. Hatfield and seconded by Dr.
Bertch-Mumaw to uphold the Board's 2007 decision designating underserved Arizona
communities for the purpose of the WICHE program. These are Apache, Graham, Greenlee,
Navajo, Mohave and Yuma counties. Motion passed, 7 ayes, 2 absent {Ms. Young and Ms.
